Routine Message: U.S. Social Security Administration – Regional Federal Benefits Unit Visit to Georgetown Guyana – Sept 14 -16, 2022

The U.S. Embassy Georgetown, Guyana is pleased to announce that representatives from the Regional Federal Benefits Unit will visit Georgetown from September 14 to 16, 2022, to offer services for beneficiaries or individuals with questions regarding U.S. Social Security and other federal benefits.

Appointments: You must schedule an appointment; no walk-in services are available. To do so, send an email to FBU.Santo.Domingo@ssa.gov, copying acsgeorge@state.gov. In the subject line write: “APPOINTMENT SSA – GUYANA”. Please include your full name, social security number, telephone number, full residency address and service you require.

Notes: Applicant’s should arrive 15 minutes before their scheduled appointment time. No large bags or computers will be allowed into the Embassy, and we cannot store these items for you if you bring them.

Alternatively, you may schedule an appointment by contacting the FBU Santo Domingo at 809-368-7011, Mondays and Thursdays from 9:00 AM to 12:00 Noon Santo Domingo time.

WHAT TO BRING FOR SOCIAL SECURITY SERVICES (please bring legible copies of all documents to be submitted):

Applying for Social Security Benefits: Bring originals and copies of the following for all applicants: Birth certificate, passport – if applying for auxiliary benefits or survivor’s benefits marriage and/or death certificates.

Social Security Number Card Application: Bring a copy of your valid U.S. passport, Certificate of Birth Abroad or original birth certificate and completed form SS-5FS.

Change of Address for Social Security: Bring your current passport.

Medicare Part B Enrollment/Cancellation: To enroll in Medicare, you should complete and sign this CMS-40B and to cancel your enrollment please complete and sign this CMS-1763.
